I, the undersigned Affiliate Member of the Dunwoody Community Garden and Orchard at Brook Run hereby agree to hold harmless Dunwoody Community Garden and Orchard at Brook Run, its Board of Directors, and owners of the land for any damage, loss, liability, claim, demand, suit, cost and expense directly or indirectly resulting from, arising out of or in connection with the use of the Dunwoody Community Garden and Orchard at Brook Run by the garden group, its successors, assigns, employees, agents and invites.
I have read and understood the Garden Bylaws on the DCGO website, www.dcgo.org and the terms of the Affiliate Membership agreement and the liability agreement above and my signature below signifies that I agree to follow the terms set forth.

I understand that the following information will be retained and used solely by the Board of Directors in order to communicate with me regarding Garden business and that this information will never be sold or shared with any other organization or entity. I agree that the following address, email and phone will be the “official” contact information for me for the receipt of any communication from DCGO. In the event any of this information changes, I will notify the Board immediately.
